- take the extra time with patients and families.
- are available 24 hours, 7 days a week putting patients and families first.
- provide all 4 levels of care.
- meet patients and families within 90 minutes of a referral (if they’re ready to meet).
- provide an Individualized Plan of Care for our patients and families, being generous with our services.
- make decisions based on the need of the patient, regardless of cost.
- offer Open Access to all eligible patients to ensure they can receive hospice benefits.
- recognize we are guests in patients’ Homes, Nursing Facilities and Assisted Living Communities.
- surround the patient/family to provide exceptional care and to determine the patients individual plan of care.
- partner with the patient’s families, facility staff, provider to deliver exceptional patient care and effective communication.
- document during the visit in our EMR to help our teammates have the information timely.
- contact families after every visit as they desire.
- attend all deaths to honor the patient, providing support to the family and facility staff.
