Description:

The Division of Long Term Services and Supports (LTSS) provides long term service options to individuals age 60 years and older and adults with disabilities age 18 years and older. In accordance with the Older Americans Act and other applicable state and federal laws, LTSS provides or purchases services for South Dakotans who meet program eligibility.

Located in each Long Term Services and Supports Office are LTSS Specialists who provide information and referral services, assistance to access in-home and community-based services, needs assessments, ongoing case management, care plan development and adult protective services to individuals living in South Dakota communities.

Services available include:

Information, Referral, and Assistance –Free objective information, referral and assistance to help individuals regardless of age, disability, income or resources identify and access public and private long-term services and supports available in South Dakota communities.

Options Planning – Free information and decision support offered to individuals, family members and caregivers to help identify long term services and supports to meet individual care needs at home and in the community.

Adult Protective Services –Assistance provided to individuals residing in the community who are at risk of abuse, neglect, exploitation, or self neglect. If you know or have reasonable cause to believe someone needs protection, please call the office listed above or local law enforcement.

Caregiver Program – Services (i.e., respite care) that help to sustain and support the caregiver as they strive to meet the needs of the person in their care.

Home and Community Based Services – Services for older adults and adults with disabilities to live in their homes or return to their homes by providing assistance with tasks they are unable to manage on their own. Services available may include: assistive devices, emergency response system, homemaker (i.e. cleaning, laundry, vacuuming), meals, nutrition, nursing, personal care (i.e. bathing, dressing), medical equipment and medical supplies.

Ombudsman Program – Ombudsman advocate to protect the rights of residents in nursing facilities and assisted living centers. Ombudsman serve as advocates for residents and investigate and work towards the resolution of resident complaints. All reports and inquiries are confidential.
