Southeastern’s Transitional Housing Program has been open for over a year and has seen tremendous growth during this time. The Transitional Housing program provides safe and stable housing and intensive coordinated services for adults who are struggling with symptoms of a mental illness and substance abuse. Participants receive case management, psychiatry services, medication management, individual and group therapy The stable housing and support allow them to focus on their health, well-being, and recovery. Southeastern partnered with the Link Community Triage Center and has opened four additional transitional units to accommodate individuals who are coming from the Link and are waiting to go to treatment.
Southeastern recently partnered with South Dakota Urban Indian HealthCare, Generation Red Road, and the Minnehaha County Sherriff’s Office to create eight additional transitional rooms for Native Americans who are involved with the criminal justice system. While living in the transitional units, participants will be provided with culturally based treatment and services.
