Avera@Home Health provides in home skilled nursing, physical therapy, occupational therapy, speech therapy, home health aide and social work services. Avera@Home staff work with physicians and members of the health care team to coordinate home care services. Our goal is to shorten hospital stays, prevent re-hospitalization, when possible, and restore and improve a person’s health. Services may be short-term following an illness or surgery or long term for individuals with chronic disease.
Home Health nurses provide: health assessment and teaching, medication administration, wound care and dressing changes, blood draws for lab work, ostomy care, Foley catheter care, IV administration and other care services ordered by the physician. Home Health aides provide personal care services under the direction and supervision of a registered nurse. They assist with activities of daily living, helping individuals to remain independent at home. Therapy services are provided in the home to assist the person to resume or restore activities of daily living. Social Workers assist individuals in managing independently in the home. Home Health staff providing care are experienced and trained.
Individuals are eligible for home health services regardless of race, gender, religion, age, national origin or economic status. Services must be ordered by a physician and the patient must leave the home infrequently. The agency is Medicare/Medicaid Certified. Payor sources for Home Health include: Medicare, Medicaid, Insurance, Veterans Administration, and self-payment.
