Description:
The Continuous Assistance, Rehabilitation and Education Program (C.A.R.E.) helps people with a severe and persistent mental illness to live successfully in the community. This program stresses outreach to consumers and the provision of services according to the individual consumer's needs, and is available through Dakota Counseling Institute's Pathway program. Each consumer is assigned a case manager who, with the active participation of the consumer, designs an individual treatment plan outlining the various services which will be provided to reach individualized treatment goals.
