- Make a transition from hospital to home
- Manage medications
- Enroll in support programs to help you stay in your home
- Regain the ability to perform everyday activities through physical therapy
- Address in-home access and safety issues with occupational therapy
- Change bandages/dressings, manage wound care and IVs or central lines
- Support newborns at high risk or people of any age with significant issues
- Handle light housekeeping, shopping, meal preparation and personal care
