Our mission is to be an open door to quality health care services, by removing barriers, especially financial, that exist for working families without health insurance or other resources to meet their health care needs. We provide complete primary health care and dental care for all age groups:

* Physical exams and preventative health care checkups
* Routine and dental care such as fillings, cleanings, extractions
* Well-child exams and immunizations
* Family planning services
* Patient education programs
* Mental health counseling and psychiatry services
* Prescription drug assistance
* HIV/AIDS early intervention services and case management.
Homeless Outreach - Taking health care outside the clinic walls. Falls Community Health provides health care to people who are homeless by assisting them to arrange appointments in the clinic or by seeing them in the community. Staff are available at outreach sites in the community. They are available to provide education and support inside and outside the clinic.
PARKING ALERT: Patient and visitor parking is located in the small lot on the north side of the building, Main Street, and Dakota Avenue.
