Avantara-Norton is a 100-bed long-term and short-term care facility which has been providing skilled nursing and rehab services to the Sioux Falls community for over 40 years. We are Medicare, Medicaid and VA Certified.
Programs and services are designed to meet the ever-changing needs of each person. An interdisciplinary team of physicians, expert healthcare professionals, and therapists work together to develop the best plan of care. Avantara-Norton has an on-site therapy department to provide services on an in-patient and out-patient basis. Memory and Dementia Care is also offered.
Beyond offering skilled nursing services, Avantara-Norton strives to build personal relationships with our residents.
Payment Sources: Self payment, insurance accepted, Medicare, Medicaid, Veterans Administration, Other: Long-term care insurance. Eligibility: 18 years of age and above and the facility must be able to meet the needs of the resident.
