Pioneer Memorial Skilled Nursing facility is a 43-bed nursing home that provides 24-hour care and focuses on individualized resident care programs that encourage family participation. We strive to promote and individualize favorite interests, memories, and routines of residents, and integrate them into social and recreational programming, enhancing the resident's quality of life. As one of few 5-star facilities in the state, our nursing care exceeds the standards of care, and includes physicians making house calls to provide care in the comfort of the resident's room.
Pioneer Haven is part of our nursing home and is a secure Dementia Unit called Pioneer Haven. This eight bed unit is specifically designed to provide a secure, calm, homelike setting for cognitively impaired residents. We also sponsor an Alzheimer's support group which is open to family and community participation, however the Haven is open to residents with a variety of diagnoses of cognitive impairments. There are also Independent, and Assisted Living and Hospital options on campus.
Eligibility Requirements: Age 62 and older and/or deemed appropriate by the State of South Dakota for long-term care placement.
